By signing this below, I understand I am responsible for all actions and consequences to myself and/or my child which may occur through training with PHX KRAV FIT / Phoenix Krav Maga And Fitness. I Do Not hold the EDS companies, Inc., PHX KRAV FIT LLC, Phoenix Krav Maga and Fitness, JP Ventures LLC, Elite Defense Systems, Matt Numrich, any staff member, or student responsible for any results rendered through any kind of martial arts training, classes, seminars, tournaments, adult firearms training, adult OC training, multi-media tools, or the like. This is also a photo/video release, as we are constantly updating our websites and promotional materials.
